Transitional Age Youth

ELIGIBILITY
• 16 to 26 years of age
• Youths age 16 to 21 years old must be Medi-Cal eligible
• Current foster youth within the Foster Care or Juvenile Justice System
• Emancipated foster youth from the Foster Care or Juvenile Justice System
• History of DCFS placement after age 16
SERVICES
• Financial assistance – rent subsidies, grocery cards, and bus pass
• Housing – securing and maintaining housing
• Internship Program
• Mentoring Program
• Job – readiness training and assistance
• Educational advocacy and support
• Independent living skills training
• Referrals and linkage to needed services
Scholarship funds provide education support for vocational and college tuition, books, fees and supplies. The Nash Scholarship Fund is a major provider of education funds for Five Acres' youth graduating from our group homes.
